Your right to an estimate of expected charges

Introduction
If you do not have health insurance, or if you choose not to use your health insurance to pay for scheduled care, federal law generally gives you the right to receive a Good Faith Estimate of expected charges.
This notice explains that right.
Your right to a Good Faith Estimate
If you are uninsured or are choosing to pay for care without submitting the services to your health plan, you generally have the right to receive a Good Faith Estimate explaining the expected cost of scheduled health care items and services.
You may also request a Good Faith Estimate before scheduling care.
A Good Faith Estimate is not a bill.
When you should receive an estimate
For care scheduled at least 3 business days in advance, federal rules generally require a Good Faith Estimate within the applicable timeframe.
If an item or service is scheduled at least 3 business days but fewer than 10 business days in advance, the estimate generally must be provided within 1 business day after scheduling.
If an item or service is scheduled at least 10 business days in advance, the estimate generally must be provided within 3 business days after scheduling.
If you request an estimate before scheduling care, it generally must be provided within 3 business days after the request.
Different rules may apply depending on the circumstances.
What the estimate includes
The estimate should identify expected charges for the reasonably expected items or services addressed by the estimate.
Actual care needs may change after an examination or as circumstances change.
The estimate does not guarantee the exact final amount of every charge.
Keep your estimate
Keep a copy of any Good Faith Estimate you receive.
You may need it if you later have questions about a bill or use the federal patient-provider dispute process.
If your bill is substantially higher
If a bill from a provider or facility is at least $400 more than the amount shown for that provider or facility on the Good Faith Estimate, you may qualify to use the federal patient-provider dispute resolution process.
